Parathyroid Adenoma
Planar sestamibi scan of the neck at
20 minutes and 2 hours shows a persistent focal region of increased uptake
separate from the thyroid gland at the level of the top of the sternum
compatible with a parathyroid adenoma (arrows
show the parathyroid adenoma; arrowheads show the thyroid gland on the
20 minute images which fades on the 2 hour images; SSN is super sternal
notch marker; a second marker is placed 10 cm above the sternal notch;
H is the heart; L is the dome of the liver). Single photon emission
computed tomography (SPECT) shows the adenoma on both cine display of the
raw data and on tomographic
sections.
Attempted excision via a neck incision was not successful. Repeat
study one month after the original study shows persistence of the parathyroid
adenoma and surgical absence of the right thyroid lobe. Reoperation
revealed a 3.4 gm, 2.2x1.5x0.9 cm parathyroid adenoma.
